Frequently Asked Questions
Housing Stabilization Services (HSS) is a new Home and Community based Services (HCBS) that supports people with disabilities in finding a stable home. The Housing Stabilization consists of three services: Housing Consultation, Housing Transitioning, and Housing Sustaining.
A member is eligible for housing stabilization services if they meet all of the following needs-based criteria:
Be on Medical Assistance (MA)
Be 18 years old or older
Proof of disability or disabling condition.
Proof of housing instability.
Proof of assessed need for services

Housing Stabilization Services provides up to 150 hours annually for housing transition support and an additional 150 hours for housing sustaining services.
A Housing Stabilization Coordinator works with individuals to identify suitable housing options and manages all necessary activities for a successful community transition.
We strive to eliminate housing barriers by securing funding sources, expunging housing records, obtaining replacement social security cards, and more.
Yes, we assist individuals find suitable housing based on their needs.
These could Include the following: Apartments, Group Homes, Provider-controlled ICS Apartments, Long Term Care Facilities, etc.
